Response of granuloma annulare to pentoxiphylline

A 68 year old man with an 18 month history of untreated biopsy-proven granuloma annulare on the trunk and extremities and normal blood glucose was started on pentoxiphylline (Trental) 400 mg. This was taken twice or three times a day for 3 months with partial resolution of the granuloma annulare. ASA 325 mg daily was added, Trental was continued, and the granuloma annulare improved to a great extent over the next 6 weeks. Granuloma annulare remained well controlled with Trental and ASA at followup after an additional 2 months.

Conclusion: ASA plus Trental may be better than Trental alone for granuloma annulare.
