A 58-year-old commercial pilot declined surgery for a 1.1 cm biopsy-proven lesion of porokeratosis on his heel, shown here after the biopsy site had healed (fig. 1). After 13 months of treatment with salicylic acid and 5-fluorouracil he was cured (figs. 2, 3 and 4). Salicylic acid (17 %) in collodion was applied under waterproof tape at night. A 1-cm diameter hole was cut in a Duoderm™ patch before it was applied in the morning. Adrucil™ (fluorouracil parenteral for IV Injection) was dropped into the hole, which was then sealed with waterproof tape. The treatment was messy but effective.
